Director of Home Care Skilled Nursing Services
Director of Home Care Skilled Nursing Services will act as a leader, decision maker, and trainer for services provided to all skilled nursing clients. This role is responsible for all clinical coordination of client care, including assessments and care plans, capability recommendations, and RN/LPN training, selection and supervision. They will act as the "lead" team member on skilled nursing clients' clinical issues and decisions. They will provide an added value to every skilled client by ensuring access to ongoing clinical supervision, training, care and quality assurance as needed.
Active Registered Nursing (RN) license in Maryland, Virginia and Washington, DC.
Strong knowledge of nursing state regulations and guidelines in areas of coverage.
Strong management, organization, and clinical skills.
Excellent communication, interpersonal, and client relation skills, both verbally and in writing.
Ability to maintain strict confidentiality among clients, families, and other health care professionals.
Strong computer skills.
Must possess a positive and enthusiastic attitude, be a team player, and possess professional demeanor and appearance.
Three years' experience in skilled nursing, hands on care.
Demonstrates proficient knowledge in medication management, tracheostomy and ventilator care, hospice services, feeding tubes, wound management, and IV infusion therapy.
Must have driver's license and reliable car for regular client visits.
Flexible schedule and hours of availability. RN is expected to be on-call for skilled services oversite.
Assist with client inquiries for prospective clients and assess appropriateness of case, as needed.
Perform detailed, skilled client initial and supervisory assessments, using a comprehensive nursing assessment format.
Create a customized Plan of Care for each client.
Effectively communicate care plan and recommendations to nursing staff, client and family.
Coordinate with client's physician and other relevant contacts regarding care plan, medications, recommendations, etc. as needed.
Participate in team meetings to discuss new clients, client/nurse issues, and prioritized tasks for the day and week. Act as "lead" on clinical decisions and issues related to client care.
Work with team to ensure client's care plan, needs, challenges, dynamics, etc. are documented, understood by all team members and passed along to nurses in an effective and thorough way. These details must be updated as the client's status changes.
Initiate, assess and investigate client accident/incident reports, as needed.
Supervise nurses in the field working with clients, including orienting to environment, reviewing care plan and documentation instructions, and facilitating relationship-building with client.
Provide training for nursing staff related to specific client needs. Ensure training is documented in staff and client records.
Coach, counsel, and discipline staff, as needed. Complete annual reviews for nursing team members.
Represent Lifematters at community meetings and events as needed.
Develop a proficient understanding and use of scheduling and assessment software, both client and nursing segments.
Attend management meetings as appropriate.
Work supportively with the Director of Clinical Services and department heads.
Develop policies and procedures as assigned. Provide written and/or verbal reports and updates on both routine and special projects.
All other duties as assigned.
Must be able to speak clear English; other languages are an additional benefit.
Must be able to hear adequately with no more than an amplifier on the phone.
Must be able to read 12 point or larger type.
