DescriptionUnder the general supervision of the Patient Care Supervisor is directly responsible for the care given to a group of patients. This includes adherence to case management principles, assessment of patient needs, development of patient care plans, scheduling of patient and team assignments, administration of physical therapy care, evaluation of physical therapy care and supervision of non-professionals and para-professionals in the delivery of patient care in the home care setting while maintaining productivity standards. In this role, the Physical Therapist is responsible for positively impacting: clinical outcomes, patient care plan goals, appropriate utilization of the agencys financial and human resources, and patient satisfaction. Physical Therapists who work a minimum of 72 hours per pay period will generally case manage 20-30 patients.

Qualifications
Minimum Requirements:

* Graduate of an approved school of Physical Therapy with a BS or MS Degree.
* Licensed by the State of California as a Physical Therapist.
* Has 2 years of appropriate clinical experience covering the full scope of licensure as a Physical Therapist.
* Documented training in: Patient assessment and evaluation is required.
* Current BLS certification.
* Performing and interpreting diagnostics test and measurements to assess pathophysiological, pathomechanical, electrophysiological, ergonomic, and developmental deficits of bodily systems to determine diagnosis, treatment, prognosis and prevention.
* Planning, administering, and modifying therapeutic interventions as part of a full practice physical therapist.
* If Memorial Medical Center and/or own vehicle used on association business, must possess valid California drivers license and upon request, provide proof of liability insurance.
* Must be able to travel in and out of Stanislaus County
* Must provide DMV printout for last three years driving history.
* The ability to read, write, hear and verbally communicate in English to the degree required to perform the job.
* Demonstrate ability to organize and present information in written and verbal format.
* Ability to manage own time and schedule own tasks.
* Initiative to work efficiently without constant supervision and direction.
* Demonstrate problem-solving skills.
* Demonstrate good organization skills.
* Demonstrate good interpersonal skills.

Preferred Qualifications:

* Membership in the American Physical Therapy Association.
* Membership in the California Physical Therapy Association.
* Has prior experience in an acute care setting.
